Reach-in cooler interior/shelves have accumulation of soil residues. Observed cooler on cook line soiled interior.
Live, small flying insects found. Observed approximately 1 small flying insect in front line area, not landing. Additionally observed 1 small flying insect in back storage area flying around, not landing.
Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed grilled chicken (40-53F - Cold Holding) in salad station cooler. As per manager item was cut was previous day, and brought to the line approximately 4 hours earlier, and stated the food fell out of temperature during service.
