Grade �A� Seats ~115 Tables are properly closed off to allow for proper social distance. Weekly extermination takes place NY State has reduced the incidence of Covid-19 by following the Governor�s executive orders regarding face coverings, social distancing, prevention of overcrowding, etc. During this pandemic, we rely on operators to follow the guidelines detailed on forward.ny.gov . Recently, your establishment was visited no violations were noted regarding the orders. Random visits will continue to occur. Thank you in advance for your cooperation. PRINTED SUMMERY OF NYS INTERIM GUIDANCE FOR FOOD SERVICE OPERATIONS DURING THE COVID-19 PUBLIC HEALTH EMERGENCY ISSUED AT THIS TIME. FOOD SERVICE ESTABLISHMENT MANAGEMENT IS REQUIRED TO ENFORCE THE PROVISIONS OF GUIDANCE FOR FOOD SERVICE DURING THE COVID-19 PUBLIC HEALTH EMERGENCY FOR BOTH CUSTOMERS AND STAFF. THE FOLLOWING WAS DISCUSSED WITH MANAGEMENT ON THIS DAY: 1) Proper mask use. 2) The requirement for daily Covid-19 health screening of all employees by questionnaire. SAMPLE EMPLOYEE COVID HEALTH SCREENING QUESTIONNAIRE ISSUED AT THIS TIME. 3) Required signage to remind both customers and staff about mask use & social distancing (signs and or floor markings required where lines form both inside and outside the facility). 4) The need for enhanced cleaning and sanitization of common surfaces on a regular schedule. 5) The requirement to notify the Nassau County Department of Health within 24 hours of learning about a positive covid-19 test result of any worker. 6) Business safety plan for operation during the covid public health emergency must be available.
